Intake Form
Fill out the online intake form to get connected to a doctor. Our service is HIPAA-compliant, so all personal and medical information is completely confidential between you and your doctor. You will NOT be charged if the doctor believes you do not qualify for a medical card.
Talk to a Doctor
The next step is to visit with a doctor visit using any video-capable device from the comfort of your home. Our doctors will work with you to create a treatment plan and provide you with all the documentation necessary to get your medical card in the State of Arkansas.
Register With the State
Once you have been certified by one of our doctors, you will need to apply online with the Arkansas Department of Health. To complete your application, you will need your Physician’s certification, your Arkansas state ID, and pay the state fee of $50. The process can take up to 14 days. Once approved your medical ID card will be mailed to you.

Qualifying Conditions
Qualifying patients must be diagnosed with one of the following conditions to be eligible for a medical identification card in Arkansas:
- Chronic Pain more than 6 months
- Cancer
- Glaucoma
- HIV/AIDS
- Hepatitis C
- ALS
- Tourette’s
- Crohn’s
- Ulcerative colitis
- PTSD
- Severe arthritis
- Fibromyalgia
- Alzheimer’s
- Cachexia or wasting syndrome
- Peripheral neuropathy
- Severe nausea
- Seizures including without limitation those characteristic of epilepsy
- Severe muscle spasms
- Any other medical condition or its treatment approved by the Department of Health
